远隔肢体缺血后适应对大鼠 MCAO 模型血管重塑的影响

摘    要:目的:研究缺血后适应对大鼠急性缺血性脑卒中脑血流恢复、脑血管直径及脑梗死体积的影响。方法将SD大鼠随机分为后适应组( MCAO+后适应)、对照组( MCAO)及假手术组( Sham)。线栓法建立大鼠永久性MCAO缺血模型,于缺血前、缺血第1天及缺血第7天对对照组和后适应组进行脑血流检测,于缺血第1天和第7天行神经功能评分,在缺血第7天行脑梗死体积检测、大脑前动脉和大脑后动脉直径检测。结果缺血第1天后适应组与对照组神经功能评分无明显差异(P>0.05),第7天实验组较对照组神经功能明显改善(P<0.05);第7天后适应组较对照组脑梗死体积明显缩小(P<0.01);第7天对照组侧支血管直径较Sham组明显增粗(P<0.01),后适应组侧支血管直径较Sham组明显增粗(P<0.01),后适应组血管直径与对照组相比明显增粗(P<0.05);缺血第1天和第7天实验组脑血流量较对照组均有明显提高(P<0.05)。结论实验结果显示缺血后适应能提高神经功能评分,减少脑梗死体积,促进缺血早期脑血流恢复,促进脑血管重塑。

Abstract:Objective To study the effect of Limb Remote Ischemic Postconditioning on cerebral blood flow restoration ,diame-ters of collateral arteries and cerebral infarction after permanent MCAO in mice .Methods SD rats were randomly divided into postcondition-ing group,ischemia control group and sham group .Suture method was used in MCAO .Cerebral blood flow was measured repeatedly at day 1 ( before and immediately after ischemia ) and day 7 at the same region .Neurological functional deficits were evaluated at day 1 after ischemia and day 7.The diameter of anterior cerebral arteries and posterior cerebral arteries were measured by a stereo -zoom microscope.And the cere-bral infarction volumes were measured at day 7.Results After 7 days,compared with the control group ,the functional neurological outcome of ischemia postconditioning group was improved significantly (P〈0.05),while on day 1 the functional neurological outcome of ischemia con-trol group and ischemia postconditioning group had no significant difference ( P〉0.05).After 7 days,compared with the ischemia control group,the infarction volumes of postconditioning group significantly reduced (P〈0.01).After 7 days,the diameters of the collateral arteries were significantly larger than the non-ischemia control group(P〈0.01),and the diameters of the ischemia postconditioning group were larger than sham group(P〈0.01).The diameters of the ischemia postconditioning group were larger than ischemia group (P〈0.05).On day 1 and day 7,cerebral blood flow of the ischemia postconditioning group was significantly improved compare with the ischemia control group ( P〈0. 05 ) .Conclusion Ischemia postconditioning can improve the functional neurological outcome and reduce the cerebral infarction volumes ,and it also can restore the cerebral blood flow earlier and mediate vascular remodeling .
