纤维乳管镜在乳头溢液性疾病术中辅助定位的临床应用

目的：探讨利用纤维乳管镜（ FDS）术中辅助定位下行病变乳管切除术的临床应用价值。方法收集2011年12月～2012年12月258例乳头溢液患者的乳管镜检查资料,对乳管镜下发现肿瘤者均要求患者行手术治疗,并比较术中乳管镜辅助定位下的病变乳管切除术和传统的乳头溢液手术的临床效果。结果经统计学分析比较,定位组在手术用时、切口长度、出血量均明显低于常规组,差异有显著性（ P＝0．002）。 FDS定位组的术后并发症和住院时间均明显低于常规组,差异有显著性（ P＝0．024）, FDS定位组的术后乳房外形恢复满意度明显高于常规组（ P＝0．001）。结论术中乳管镜辅助定位下行病变乳管切除术可以精确地找到病变位置,明确发病原因,且手术切除范围小,病理检出率高,疤痕小,术后美容效果好。

Fiberoptic Ductoscopy in Nipple Discharge Diseases in the Clinical Application of Auxiliary Positioning

Objective To investigate the diagnostic value of positioning degeneration breast mass resection using fiberoptic ductoscopy （ FDS） in the operations and its clinical application .Methods From December 2011 to December 2012,FDS were performed in a total of 258 patients with nipple discharge in Weifang Traditional Chinese hospital and subsequently all tumor discovered were asked surgically removed .The FDS findings were compared with the results of traditional nipple discharge operations .Results By statistical analysis,the operation time,incision length and bleeding volume of positional group were significantly lower than the normal group .The difference was statistically significant （ P=0 .002 ） .The complications and hospital stay of FDS positional group were significantly lower than the normal group.The difference was statistically significant （P=0.024）.Additionally,the satisfaction degree of breast recovery of FDS positional group was obviously lower than that of normal group （P=0.001）.Conclusion Positioning degeneration breast mass resection using FDS in operations could precisely find the lesion position and etiology with the least opera -tional resection and the highest pathological detection rate .No large range of scar would be leaved after the operation due to its good cosmetic effect .
