摘    要:目的:探讨动脉自旋标记(ASL)在DWI和MRA阴性临床诊断为短暂性脑缺血发作(TIA)患者中的脑血流灌注价值。方法对常规检查阴性的TIA患者78例,于末次发作24 h内进行 ASL灌注检测,3名观察者分别观察ASL 局部脑血流量( CBF)伪彩图显示灌注异常并进行相对局部脑血流量图( rCBF)测量。结果3名观察者独立观察并共同分析78例ASL CBF图并测量ASL-rCBF得到:低灌注42例(53.85%),其中TIA临床表现与低灌注区对应36例,符合率为87.50%.结论 ASL灌注对临床诊断为TIA的影像学诊断有极大的作用,为临床诊断提供影像学证据。

Abstract:Objective To study the application value of arterial spin-labeling in cerebral perfusion of patients with TIA with negative diffusion and vascular imaging presenting clinically .Methods The perfusion detection of ASL was acquired on 78 patients diagnosed of TIA within 24 hours of last symptom onset .All patients had negative findings on general MR .The ASL CBF pcolor was obsevered on the basis of perfusion disturbance and the rCBF was meas -ured by 3 independent observers .Results There were 42 patients(53.85%) of the 78 ASL CBF maps reading with perfusion deficits.With the 36 patients the perfusion deficits were matched with the clinical symptoms ,and the compliance rate was 84 .62%.Conclusion ASL perfusion shows promise in the detection of perfusion deficits that correlated with clinically diagnosed TIA patients ,and provides imaging evidence for the clinical diagnosis .
