低位直肠癌手术中双吻合器的应用体会

目的探讨双吻合器在低位直肠癌直肠前切除术中的应用及并发症发生的原因与防治.方法回顾分析87例低位直肠癌应用双吻合器行直肠前切除术的情况,总结经验教训.结果大部分手术顺利,无手术死亡,术后发生吻合口瘘5例(5.7%),吻合口狭窄1例(1.1%),均治愈.结论双吻合器的使用提高了低位直肠癌保肛手术的成功率,完善的术前准备、正确的手术操作可有效地预防并发症的发生.

Double stapline technique in surgical treatment of low rectal cancer

Objective To evaluate the role of double-stapling technique i n low anterior colorectal resection, investigate causes of the complications and bring forward corresponding prevention and cure measures. Methods A retrosp ective study was conducted on 87 consecutive patients who underwent double-stapl ed anterior resections for low rectal carcinoma. Results Most of the 87 case s of double stapling procedure were successful without operative deaths. Correla tive complications including anastomotic fistula in 5 cases (5.7%) and anastomot ic stenosis in 1 case (1.1%) were cured through non-surgical treatment. Conclu sions In comparison with traditional hand-sewn Dixon, the double-staple techni que can greatly improved the feasibility of anus-preserved operation for low and ultra-low rectal cancer. Perfect preoperative preparation, especially the norma tive operation and correct use of double stapling technique can effectually decr ease the complications.
