老年患者心脏手术围手术期主动脉内球囊反搏的应用

摘    要:目的探讨老年高危冠心病患者心脏手术围手术期应用主动脉内球囊反搏(IABP)的效果。方法对我院1998年1月~2006年6月入院的98例(年龄≥65岁)冠心病(冠心病组)和20例非冠心病(非冠心病组)患者心脏手术围手术期应用IABP的资料进行总结分析。结果118例患者死亡37例,总病死率为31.36%,其中冠心病组患者死亡23例,病死率为23.47%,非冠心病组患者死亡14例,病死率为70.00%。术前应用IABP的冠心病患者死亡2例,病死率为6.45%,明显低于术中和术后应用IABP的冠心病患者的病死率12例(30.00%)和9例(33.33%)]。结论老年高危冠心病患者心脏手术围手术期应用IABP是一种安全有效的辅助循环方式,可以降低病死率。

Abstract:Objective To explore the indication and application of intra-aortic balloon pump(IABP) in the elderly high-risk patients undergoing cardiac operations.Methods From year 1998 to 2006,98 elderly patients with CAD and 20 elderly patients with non-CAD underwent cardiac operation using IABP.Results The total fatality rate was 31.36%,the fatality rate in coronary heart disease patients was 23.47%,while that in non-coronary heart disease patients was 70.00%.In coronary heart disease patients using IABP preoperatively,the fatality rate was 6.45%,which was much lower than that in those using IABP during operation or postoperatively(30.00%,33.33% respectively).Conclusion IABP is a safe and effective form of circulation support in the elderly.It is beneficial to use IABP preoperatively in high risk coronary heart disease patients.
