细胞因子与孕期B族链球菌亚临床感染

目的探讨以白细胞介素-6(interleukin,IL-6)I、L-8及其受体水平,作为早期诊断B族链球菌(GBS)亚临床感染指标的可行性。方法对208例孕妇进行GBS培养及其他病原体检测,对其中无其他病原体感染的80例(GBS阳性20例,阴性60例)孕妇留取母血、羊水、脐血,用酶联免疫吸附法和免疫组化法行细胞因子及其受体检测。结果208例孕妇中GBS阳性20例,占9.62%。GBS带菌孕妇母血、羊水中的IL-6I、L-8明显高于正常孕妇(P<0.05,P<0.01);绒毛膜羊膜炎时母血、羊水IL-6I、L-8质量浓度明显增加(P<0.05,P<0.01),GBS亚临床感染时母血、羊水中IL-6I、L-8质量浓度也较对照组明显增加(P<0.05,P<0.01)。结论将母血IL-6作为预测GBS亚临床感染的有效指标有很大的临床价值。

The Relationship between Cytokine and Group B Streptococcus Subclinical Infection in Pregnancy

Objective To determine whether the Interleukin-6(IL-6),and Interleukin-6 Recep-tor(IL-6R),and Interleukin-8(IL-8) concentrations can be used as index for the diagnosis of GBS subclinical infection in early pregnancy.Methods Culture sepcimens for group B streptococcus were obtained from 208 pregnant women.The maternal serum,amniotic fluid and cord blood were collected seperately in selected 80 pregnants(20 pregnants without infections of other pathogens were GBS carriers;60 pregnants were controls),IL-6 and IL-8 levels were detected by enzymy-linked immunosorbent assay(ELISA) and immunohistochemistry technique.Results There were 20 GBS carriers in 208 pregnants(9.62%).The maternal serum and amniotic fluid IL-6,IL-8 levels in GBS carriers were higher than those in control group.The maternal serum and amniotic fluid IL-6,IL-8 in patients complicated with chorioamnionitis or GBS subclinical infection were all higher than those in control group.Determining cytokine was valuble clinical index for indentification of the chorioamnionitis in patients with GBS subclinical infection,but the relation between cytokine and the incidence of newborn was not found.Conclusion The maternal serum IL-6 is valuable clinical index for indentification of the patients with GBS subclinical infection.
