复合式小梁切除术联合生物羊膜植入在闭角型青光眼中应用

目的探讨复合式小梁切除术联合生物羊膜植入治疗闭角型青光眼的临床疗效。方法回顾性分析2012年4月至2013年4月我院收治的闭角型青光眼共40例（40只眼）,采用复合式小梁切除术联合生物羊膜植入16例（观察组）,传统小梁切除术24例（对照组）。分析比较两组术后滤过泡、眼压、前房形成及并发症等情况。结果 40只眼手术顺利,术后随访1年.术后第1d对照组浅前房发生率24%,观察组为6.25%（P〈0.05）。末次随访,对照组20只眼压控制在6-21 mm Hg（62.5%）,观察组15只眼眼压控制在6-21 mm Hg（87.5%）,对照组功能性滤过泡占62.5%,观察组占87.5%,两组间比较差异有统计学意义（P〈0.05）结论复合式小梁切除术联合生物羊膜植入可有效降低眼压,且并发症少,是治疗闭角型青光眼安全、有效的手术方法之一。

Application of trabeculectomyandamniotic membrane implantation in angle closure glaucoma

Objective To investigate the clinical effects of combined trabeculectomy and amniotic membraneimplantation for the treatment of angle closure glaucoma. Methods We retrospectivelyreviewed the medical records of40patients（40 eyes） with angle-closure glaucoma. Patients were randomly divided into two groups. Sixteen patients（16eyes）underwent combined trabeculectomyand amniotic membrane implantation,and 24 patients（24 eyes） received trabeculectomy only as control. Postoperative intraocular pressure（IOP）,bleb filtration,anterior chamber depth,and surgical complications were compared between thetwo groups. Results The incidence of shallow anterior chamber was 6. 25% inpatients received amniotic membrane implantation,and was 24% in control group（ P〈 0. 05） on thefirst postoperative day,respectively. After 12 months,87. 5% of the patients who received amniotic membrane implantation maintained an intraocular pressure between 6 and 21 mmHg,while the rate was only 62. 5% in the control group（ P〈 0. 05）. Functioning filtering blebs were observed in 87. 5% and 62. 5% of the patients in the two groups,respectively（ P 〈0. 05）. Conclusion Combined trabeculectomywith amniotic membrane implantation is and effective and safe way to treat angle-closure glaucoma.
