开腹与腹腔镜手术治疗60例结直肠癌的临床对比研究

目的对比开腹和腹腔镜下结直肠癌根治术的临床疗效。方法将60例结直肠癌患者随机分成两组,开腹组30例,实施开腹结肠癌根治术;腹腔镜组30例,实施腹腔镜下结直肠癌根治术。结果开腹组术中平均出血量（235±20）mL,腹腔组术中平均出血量（145±19）mL,开腹组术中出血量明显多于腹腔镜组（P〈0.05）;开腹组胃肠功能恢复平均2.5d,腹腔组胃肠功能恢复平均1.5d,开腹组胃肠功能恢复时间明显多于腹腔镜组（P〈0.05）;开腹组手术时间平均（165±32）min,腹腔组手术时间平均（150±45）min,两组手术时间无差异（P〉0.05）;术后两组患者均住院平均7d;术后均未出现吻合口漏、出血和狭窄等并发症。结论开腹与腹腔镜下结直肠癌根治术在根治彻底性、术后远处转移与局部复发方面作用无差异,腹腔镜手术治疗具有创伤小、恢复快的特点。

Study on clinical effect of iaparotomy and iaparoscopic surgery for colorectal cancer

Objective Comparative study on Clinical curative effect of laparotomy and laparoscopic surgery for Colorectal cancer.Methods 60 cases of Colorectal cancer patients were randomly divided into two groups, laparotomy group 30 cases, the implementation of open radical resection of colon cancer; laparoscopy group 30 cases, the implementation of laparoscopic radical resection of colon cancer. Results The average abdominal bleeding volume in operation of (235±20)mL , the average intraperitoneal bleeding volume in operation of (145±19)mL, open bleeding volume in operation significantly more than the laparoscopic group (P < 0.05); The average gastrointestinal function recovery time of laparotomy group were 2.5d, the average gastrointestinal function recovery time of laparoscopic group were 1.5d, gastrointestinal function recovery time of laparotomy group significantly more than the laparoscopic group (P < 0.05); The average operation time of laparotomy group were (165±32)min, the average operation time of laparoscopic group were(150±45)min, two groups of operation time no difference (P > 0.05); The average length of time of two groups were 7d; There were no complications after operation,such as bleeding and stenosis complication. Conclusion Radical thoroughness, distant metastasis and local recurrence of laparotomy and laparoscopic surgery for colorectal cancer no difference, characteristics of laparoscopic surgery were less trauma, rapid recovery characteristics.
