基底细胞样乳腺癌组织中E-钙黏蛋白及波形蛋白的表达及临床意义

目的研究基底细胞样乳腺癌(BLBC)组织中E-钙黏蛋白及波形蛋白的表达及临床意义。方法选取2016年3月-2017年7月在该院接受手术治疗的98例BLBC患者的癌组织切除标本(BLBC组),同时取BLBC患者距离癌灶边缘5 cm的正常乳腺组织标本作为对照组,另选同期在该院接受手术治疗的95例非BLBC患者的癌组织切除标本记为NBLBC组。检测并对比各组E-钙黏蛋白及波形蛋白的表达,分析BLBC患者E-钙黏蛋白及波形蛋白的表达与患者病理特征的关系以及E-钙黏蛋白与波形蛋白在BLBC患者组织中的表达关系。结果 BLBC组和NBLBC组的E-钙黏蛋白阳性表达率均明显低于对照组,且BLBC组明显低于NBLBC组,差异均有统计学意义(均P<0. 05)。BLBC组和NBLBC组的波形蛋白阳性表达率均明显高于对照组,且BLBC组明显高于NBLBC组,差异均有统计学意义(均P<0. 05)。TNM分期为Ⅲ～Ⅳ期和有淋巴结转移者的E-钙黏蛋白阳性表达率分别低于Ⅰ～Ⅱ期和无淋巴结转移者,而波形蛋白阳性表达率分别高于Ⅰ～Ⅱ期和无淋巴结转移者,差异均有统计学意义(均P <0. 05)。根据Spearman相关性分析可知,E-钙黏蛋白与波形蛋白在BLBC患者组织中的表达呈负相关(r=-0. 557,P<0. 05)。结论 E-钙黏蛋白在BLBC患者组织中的阳性表达明显降低,而波形蛋白的阳性表达则明显升高,二者均与BLBC患者的临床分期及癌症转移密切相关,临床上可考虑将二者作为新型监测靶点,有助于辅助临床诊治过程。

Expression and clinical significance of E-cadherin and vimentin in basal-like breast carcinoma

Objective To explore the expression and clinical significance of E-cadherin and vimentin in basal-like breast carcinoma(BLBC).Methods Cancerous tissue excision specimens from 98 patients with BLBC who underwent surgical treatment in the Second Clinical Medical College of Ji'nan University from March 2016 to July 2017 were recorded as BLBC group,at the same time,the normal breast tissue samples from the BLBC patients within 5 cm from the edge of lesion were taken as control group,and cancerous tissue excision specimen from 98 patients with non BLBC who underwent surgical treatment in the hospital during the same period were recorded as NBLBC group.The expression levels of E-cadherin and vimentin in the three group were detected and compared,the relationships between the expression levels of E-cadherin and vimentin in patients with BLBC and pathological features of the patients were analyzed,and the relationship between E-cadherin expression and Vimentin expression in the tissues of patients with BLBC was analyzed.Results The positive expression rates of E-cadherin in BLBC group and NBLBC group were statistically significantly lower than those in control group,the positive expression rate of E-cadherin in BLBC group was statistically significantly lower than that in NBLBC group(P<0.05).The positive expression rates of vimentin in BLBC group and NBLBC group were statistically significantly higher than those in control group,the positive expression rate of vimentin in BLBC group was statistically significantly higher than that in NBLBC group(P<0.05).The positive expression rates of E-cadherin in patients with TNM stage Ⅲ-Ⅳ and lymph node metastasis were statistically significantly lower than those in patients with TNM stageⅠ-Ⅱ and without lymph node metastasis,and the positive expression rates of vimentin in patients with TNM stage Ⅲ-Ⅳ and lymph node metastasis were significantly higher than those in patients with TNM stage Ⅰ-Ⅱ and without lymph node metastasis(P<0.05).Spearman correlation analysis showed that E-cadherin expression was negatively correlated with vimentin expression in the tissues of patients with BLBC(r=-0.557,P<0.05).Conclusion The positive expression of E-cadherin in the tissues of patients with BLBC significantly decreases,and the positive expression of vimentin significantly increases,the two factors are closely related to clinical staging and cancer metastasis of patients with BLBC.It is possible to consider the two factors as new monitoring targets in clinic,which helps to assist clinical diagnosis and treatment.
