枸橼酸咖啡因治疗早产儿脑损伤的价值分析

摘    要:目的探讨枸橼酸咖啡因治疗早产儿脑损伤的价值,为早产儿脑损伤的治疗提供理论依据。方法选取2014年3月-2016年3月自贡市第一人民医院收治的早产儿55例为研究对象,根据是否使用枸橼酸咖啡因分为对照组(23例)和观察组(32例),对比两组间头颅MRI、脑电图(EEG)、NBNA评分、神经元特异性烯醇化酶(NSE)浓度治疗前后变化。结果纠正胎龄40周时,头颅MRI及EEG结果均显示,观察组有效率高于对照组,差异有统计学意义(P<0. 05);观察组NBNA评分高于对照组,NSE浓度低于对照组,差异有统计学意义(P<0. 05)。结论枸橼酸咖啡因可以改善早产儿脑损伤头颅MRI、EEG结果,提高NBNA评分,降低NSE浓度,可作为一种新型治疗手段应用于早产儿脑损伤。

Abstract:Objective To explore the value of caffeine citrate in treatment of brain injury in preterm infants,provide a theoretical basis for treatment of brain injury in preterm infants.Methods Fifty-five preterm infants treated in the First People's Hospital of Zigong from March 2014 to March 2016 were selected and divided into control group(23 infants) and experimental group(32 infants) according to use of caffeine citrate.Head MRI findings,electroencephalogram(EEG) findings,NBNA scores,neuron-specific enolase(NSE) levels before and after treatment in the two groups were compared.Results At 40 weeks of corrected gestational age,head MRI and EEG showed that the effective rate in experimental group was statistically significantly higher than that in control group(P<0.05),NBNA score in experimental group was statistically significantly higher than that in control group(P<0.05),NSE level in experimental group was statistically significantly lower than that in control group(P<0.05).Conclusion Caffeine citrate can improve the results of head MRI and EEG of preterm infants with brain injury,improve NBNA score,and reduce NSE level,which can be used as a new treatment for brain injury in preterm infants.
