脂联素基因多态性与妊娠期糖尿病的相关性

摘    要:目的分析脂联素(APN)基因单核苷酸多态性(SNP)位点rs2241766 (+45T/G)、rs1501299 (+276G/T)和rs16861194 (-11426A/G)与福建地区妊娠期糖尿病(GDM)的相关性,探讨APN在福建地区GDM发病中的作用。方法采用病例-对照研究,选取2015年10月-2016年9月在福建省妇幼保健诊断为GDM的孕妇150例为GDM组,同期正常妊娠孕妇150例为对照组。采用ELISA法检测两组孕妇血清APN和胰岛素水平,直接测序法检测APN基因位点多态性。分析比较两组血清APN水平、APN基因位点基因型和等位基因频率的差异。结果所选APN基因3个SNP位点均符合Hardy-Weinberg平衡检验(P>0. 05)。GDM组血清APN水平明显低于对照组,而空腹胰岛素(FINS)和HOMA-IR水平明显高于对照组,差异有统计学意义(P<0. 05)。两组+45T/G位点和+276G/T位点基因型及等位基因频率比较差异无统计学意义(P>0. 05); GDM组-11426A/G位点AG和GG基因型检出率均高于对照组,GDM组G等位基因突变频率高于对照组,差异均有统计学意义(P<0. 05)。GDM组-11426A/G位点AG和GG基因型患者血清OGTT 1 h、OGTT 2 h、FINS和HOMA-IR水平均高于AA基因型患者,且GG基因型患者高于AG基因型; AG和GG基因型患者血清APN水平低于AA基因型,GG基因型患者血清水平低于AG基因型,差异均有统计学意义(P<0. 05)。结论 APN基因-11426A/G位点多态性及血清APN水平的降低可能与福建地区GDM发生有关。

Correlation between adiponectin gene polymorphism and gestational diabetes mellitus

Abstract:Objective To analyze the correlations between adiponectin gene polymorphism [rs2241766(+ 45 T/G),rs1501299(+276 G/T),and rs16861194(-11426 A/G) ]and gestational diabetes mellitus(GDM) in Fujian,explore the role of adiponectin in occurrence of GDM in Fujian.Methods A case-control study was conducted,150 pregnant women diagnosed as GDM in Fujian Provincial Maternal and Child Health Care Hospital from October 2015 to September 2016 were selected as GDM group,150 normal pregnant women during the same period were selected as control group.ELISA was used to detect the levels of serum adiponectin and insulin in the two groups.Direct sequencing method was used to detect adiponectin gene polymorphism.Serum adiponectin level,adiponectin gene loci genotypes and alleles in the two groups were analyzed and compared.Results All the three SNPs were in accordance with Hardy-Weinberg equilibrium(P >0.05).Serum adiponectin level in GDM group was statistically significantly lower than that in control group,and the levels of fasting insulin(FINS) and HOMA-IR were statistically significantly higher than those in control group(P<0.05).There was no statistically significant difference in the frequencies of adiponectin + 45 T/G and + 276 G/T genotypes and alleles between the two groups(P >0.05).The detection rates of AG and GG genotypes of-11426 A/G were statistically significantly higher than those in control group(P<0.05).The levels of one-hour OGTT,two-hour OGTT,FINS,and HOMA-IR in patients with AG and GG genotypes of-11426 A/G locus in GDM group were statistically significantly higher than those in patients with AA genotype,and which were higher in patients with GG genotype compared with patients with AG genotype.The levels of adiponectin in patients with AG and GG genotypes was statistically significantly lower than that in patients with AA genotype,the level of adiponectin in patients with GG genotype was statistically significantly lower than that in patients with AG genotype(P<0.05).Conclusion Adiponectin gene-11426 A/G polymorphism and decrease of serum adiponectin level may be correlated with occurrence of GDM in Fujian.
