我院住院患者特殊使用级抗菌药物应用分析

目的了解特殊使用级抗菌药物(以下简称特抗)的应用特点,为特抗的合理应用及科学管理提供参考依据。方法从医院HIS系统中查询2016年8月～2017年8月特抗的使用数据,分析特抗的人群特点、科室分布、感染指标、病原学检查情况和联合用药合理性等情况。结果 2016年8月～2017年8月我院用量最大的特抗为美罗培南,其DDDs值为6408.25,使用特抗最多的科室为重症医学科,其DDDs值为4576.75。使用特抗的1316例患者中男性、老年人居多;70%以上为呼吸系统感染;75%以上存在体温升高;WBC升高者占42.63%;NEU%升高者占64.06%;病原学送检率占83.66%。结论我院使用特抗患者体温及血液学指标基本能够反映感染状况,微生物送检率符合卫生部的要求,但也存在部分问题,需要各学科共同参与,进一步严格管控。

Analysis of the application of special-grade antibacterial drugs in hospitalized patients in our hospital

Objective?To understand the application characteristics of special-purpose antibacterial drugs (hereinafter referred to as special antibiotics),and provide reference for the rational application and scientific management of special antibiotics.Methods?The data of the use of special antibiotics from August 2016 to August 2017 were inquired from the hospital HIS system.The characteristics of the population,the distribution of the department,the infection index,the pathogen examination and the rationality of the combined medication were analyzed.Results?From August 2016 to August 2017,the maximum consumption special antibiotic was meropenem in our hospital,and its DDDs value was 6408.25.The department with the most special antibiotics was in the Department of Critical Care Medicine,and its DDDs value was 4576.75.Among the 1316 patients who used special antibiotics,males and the elderly were the majority;more than 70% were respiratory infections;more than 75% had elevated body temperature;42.63% had increased WBC;64.06% had increased ENU%;the etiology inspection rate accounted for 83.66%.Conclusion?The body temperature and hematology indicators of patients with special antibiotics can basically reflect the infection status.The microbiological examination rate meets the requirements of the Ministry of Health.But there are also some problems,which require the participation of various disciplines and further strict control.
