| Abstract: | Exploration of the extrahepatic biliary passages are commonly required to relieve obstructive jaundice caused by gallstones, tumors of the common duct, A long, vertical, upper right transrectus or paramedian incision is the one most commonly used. It should be ample and provide a wide exposure. The abdominal cavity is entered. Explore the abdomen. Examine the liver and gallbladder. The assistant'sleft hand, or a retractor, draws the duodenal bulb downwards. A second retractor draws the liver upward, thus. opening up the space between liver and duodenum. Display and palpate the cystic duct, hepatic duct, common hepatic duct and the common bile duct. If a stone can be palpated within the common bile duct an incision is made just long enough to permit the escape of the presenting stone. According to our account, the common bile duct is often crossed anteriorly by the cystic artery, portal vein, aberrant hepatic artery. It is probable that these arteries can be incised at choledochostomy. |
