脑星形细胞肿瘤病理分级与DI、5cER及MRI表现的相关性研究

摘    要:目的: 研究脑星形细胞肿瘤的DI、5cER及MRI表现与其恶性程度间的相关关系,为临床上判断星形细胞肿瘤患者的预后提供一些新的依据。 方法: 40例星形细胞肿瘤石蜡标本经Feulgen染色后,用图像分析仪检测其DNA指数(DI)及超5倍体率(5cER);复习患者MRI表现,包括:瘤周水肿程度、边界是否清楚、信号是否均匀、病灶是否明显强化、病灶是否过中线、有无囊变坏死6项指标,与肿瘤病理分级诊断及生存率相比较。 结果: DI、5cER、MRI表现与病理分级诊断均有显著的正相关关系;与生存率之间有显著的负相关关系。 结论: DI、5cER及MRI表现对星形细胞肿瘤恶性度及患者预后的判断具有重要的参考价值,5cER平均值超过20时,应考虑为高度恶性星形细胞肿瘤。

Abstract:AIM: To study the correlation between DNA index(DI),5cER,MRI appearance and the malignant grades of brain astrocytic tumors.METHODS: After Feulgen staining,40 paraffin samples of astrocytic tumors were observed about DI and 5cER by image analysis,MRI appearances were reviewed,including edema around tumor,clearity of boundary,equality of MRI singal,enhancement of tumor,shift of medline,necroses and cyst,then compared them with pathological grade and survival rate.RESULTS: There was a significant positive correlation between DI,5cER,MRI appearance and pathological grade,negative correlation between DI,5cER,MRI appearance and survival rate.CONCLUSION: DI,5cER and MRI appearance are very important in judging pathological grade and patient's prognosis.When 5cER mean of astrocytic tumor exceeds 20,the tumor should be judged as glioblastoma.
