多层螺旋CT二维及三维重建在髋臼骨折诊断中的价值

摘    要:[目的]评价多层螺旋CT二维及三维重建在髋臼骨折诊断中的价值.[方法]对10例髋臼骨折患者行螺旋CT轴向扫描和二维、三维重建,分析图像表现,对骨折进行分类.[结果]轴向CT扫描可以较好地显示骨折,尤其是关节内骨折碎片的存在,但是横行骨折在轴向CT扫描图像上不易显示,而三维CT重建图像可以帮助医师准确地了解骨折线的走向,关节面的损伤程度以及骨折碎片的空间分布情况,完善术前计划.然而三维CT重建对无移位的髋臼骨折显示不满意,二维重建(MPR)是对其有效的补充.[结论]对于有移位的髋臼骨折尤其是复杂的骨折,应尽可能行二维及三维CT重建协助完善术前准备,三维CT对无移位的髋臼骨折实用价值不大.

Abstract:[Objective] To evaluate the value of 2D rconstruction and three-dimensional multislice spiral CT in the diagnosis of acetabular fractures.[Methods] A total of 10 patients with acetabular fractures were evaluated by axial computed tomography(CT),2D rconstruction and three-dimensional CT.[Results] Axial CT scans readily demonstrated the fractures and presence of intraarticular fragments.But transverse fracture isn't easily showd.3D CT was helpfull in understanding the precise plane of the fracture,the degree of disruption of disruptions of the articular surface,and spatial relationships of fragments.But the 3D CT images were less satisfactory for undisplaced fractures.multi planar reconstruction(MPR) was the valid complement for it.[Conclusion] Complex acetabular fractures with displacement should be evaluated by means of 2D rconstruction and three-dimensional multislice spiral CT.However 3D CT is of little practical value in undisplaced acetabular fractures.
