突发性聋早期与远期疗效的关系及临床意义

目的探讨突发性聋治疗后早期与远期疗效的关系,以便为突发性聋疗效评估及挽救性治疗实施时机提供理论依据。方法分析88例突发性聋患者在治疗开始后的第3、7、14天和第1、2、3、4、6个月的听力情况。结果治疗后第3天和第7天总有效率与其他时间点比较及第14天与第2、3、4、6个月比较差异有统计学意义;治疗后第1个月与第2、3、4、6个月比较,总有效率有较大的波动,2个月后进入一个平台期。治疗后第3-14天,治愈率、显效率和有效率构成数比值呈上升趋势,第14天后基本稳定。结论①治疗第14天内的听力可对总的疗效趋势作出大致判断,但不能作为疗效评估的终点;②治疗2个月后突发性聋疗效进入一个稳定期,此时行疗效评估更加客观和准确。

Relationship between early and long-term curative effect and its clinical significance in sudden deafness

OBJECTIVE To explore the relationship between early and long-term curative effect in sudden deafness so that the theoretic evidence can be provided to choose the best time to evaluate therapeutic effects and salvage therapy plan.METHODS Pure tone audiometries were performed in 88 cases of sudden deafness patients before treatment and on the third day,seventh day,fourteenth day,the first month,second month,third month,fourth month and sixth month after treatment.All of acoustical data were analyzed.RESULTS The total effective rates were statistically different between the third,seventh post-treatment day and the other time point.Statistical differences were found between the fourteenth day and the second,third,fourth,sixth month after treatment.If the total effective rates were compared between the first and the second,third,fourth,sixth month,it was found fluctuating in the frst month,and get to the plateau stage after the second month.The constituent ratio of the curative rates,the excellent rates,the effective rates were found to be upward from the third to fourteenth day after treatment,and it get to the plateau stage after the fourteenth post-treatment day.CONCLUSION 1.Prognosis can be predicted approximately 2 weeks after start of treatment,But the hearing results couldn＇t be taken as terminal hearing level to evaluate curative effect.2.The time of stabilization of curative effect was 2 months after treatment.It would be more objective and accurate to evaluate the curative effect at this point of time.
