耳鼻咽喉头颈外科住院总医师气管切开分析

目的分析担任耳鼻咽喉头颈外科住院总医师期间气管切开病例的构成特点，从一个侧面反映住院总医师的工作情况。方法回顾性分析第四军医大学附属西京医院耳鼻咽喉头颈外科1名住院总医师在2009年9月1日-2010年9月1日期间行气管切开术的患者。结果113例患者气管切开成功率100％，无一例出现严重并发症，无死亡病例，发生造瘘口出血17例，造瘘口皮下气肿21例。结论气管切开是挽救患者性命的重要手段，但术中、术后均可能发生一定的并发症，严重的可造成患者的死亡，因此气管切开术是担任耳鼻喉科住院总医师期间必须要熟练掌握的技能。

Tracheotomy by an otolaryngology chief resident

OBJECTIVE To analyze the clinical characteristics of patients who underwent tracheotomy by an otolaryngology chief resident. METHODS The clinical data of the patients who underwent tracheotomy by an otolaryngology chief resident in Department of Otolaryngology, Xijing Hospital of The Fourth Military Medical University from Sep, 1, 2009 to Sep, 1, 2010, were retrospectively studied. RESULTS Tracheotomy was successfully performed in 113 patients. Postoperative bleeding occurred in 17 cases, subcutaneous emphysema in 21 cases. No severe postoperative complications and died cases occurred. CONCLUSION Tracheotomy is an important ways for saving the patients＇ life, but intraoperative and postoperative complications may occur. Severe complications may cause the death of the patients. So the tracheostomy is a skill that an otolaryngology chief resident must master.
