冻融胚胎卵裂球损伤对胚胎发育潜能的影响

摘    要:目的:探讨冻融胚胎卵裂球完整性对胚胎发育潜能的影响。方法:回顾性分析126个冻融胚胎移植周期,按照移植胚胎的卵裂球的完整性分为两组:无损伤组(所有移植胚胎均完整无损,n=78),部分损伤组(移植胚胎中至少含1个有卵裂球损伤的胚胎,n=48),比较两组临床妊娠率、种植率和流产率。结果:无损伤组临床妊娠率为37.18%(29/78),种植率23.72%(37/156),流产率3.45%(1/29)。部分损伤组临床妊娠率为33.33%(16/48),种植率13.85%(18/130),流产率12.50%(2/16),两组临床妊娠率和流产率比较,差异无统计学意义(P>0.05),种植率比较差异具有统计学意义(P<0.05)。结论:部分卵裂球损伤的冻融胚胎,其种植潜能降低,卵裂球完整性是影响临床结局的关键性因素。

Effect of blastoomere injury of frozen-thawed embryo on developmental potential of embryo
Abstract:Objective:To explore the effect of blastomere integrity of frozen-thawed embryo on developmental potential of embryo.Methods:A total of 126 frozen-thawed embryo thansfer cycles were analyzed retrospectively,then they were divided into two groups according to blastomere integrity: no-injury group(all the embryos were intact,n=78) and partial injury group(including at least one damaged embryo among all the embryos,n=48);the clinical pregnancy rates,implantation rates,and abortion rates in the two groups were compared.Results:The clinical pregnancy rate,implantation rate,and abortion rate in no-injury group were 37.18%(29/78),23.72%(37/156),and 3.45%(1/29),respectively;the clinical pregnancy rate,implantation rate,and abortion rate in partial injury group were 33.33%(16/48),13.85%(18/130),and 12.50%(2/16),respectively.There was no statistically significant difference in clinical pregnancy rate and abortion rate between the two groups(P>0.05),but there was statistically significant difference in implantation rate between the two groups(P<0.05).Conclusion:The implantation potential of partial frozen-thawed embryos with blastomere injury decreases,integrity of blastomere is one of key factors affecting clinical outcome.
