心理干预对不同手术方式子宫肌瘤患者术后生活质量及婚姻质量的影响

目的:探讨心理干预对不同手术方式子宫肌瘤患者术后生活质量及婚姻质量的影响。方法:将入住南昌大学第一附属医院妇产科的400例子宫肌瘤患者随机分为干预组和对照组,采用诺丁汉健康调查问卷及Olson婚姻质量调查问卷分别于术前和术后6个月对患者的生活质量及婚姻质量进行评估。结果:①手术后,干预组中各组的生活质量评分明显低于对照组相应各组,差异有统计学意义(P<0.05);对照组中全子宫切除组的生活质量评分较术前明显升高,次全子宫切除组、肌瘤剔除组及血管介入组的生活质量评分较术前明显降低,差异有统计学意义(P<0.05)。②手术后,干预组中各组的婚姻质量评分明显高于对照组相应各组,差异有统计学意义(P<0.05);对照组中全子宫切除组的生活评分较术前明显降低,次全子宫切除组、肌瘤剔除组及血管介入组的生活质量评分较术前明显升高,差异有统计学意义(P<0.05)。结论:子宫肌瘤全子宫切除术患者的生活质量和婚姻质量显著下降,心理干预对于子宫肌瘤患者术后生活质量及婚姻质量的改善有重要作用。

Effect of psychological intervention on the quality of life and marital quality of patients with uterine fibroid after different surgical methods

Objective:To explore the effect of psychological intervention on the quality of life(QOL) and marital quality of patients with uterine fibroid after different surgical methods.Methods:Four hundredn patients with uterine fibroid who were treated in department of gynecology and obstetrics,the first affiliated hospital of Nanchang university were randomly divided into intervention group and control group,Nottingham Health Questionnaire and Olson Mariral Quality Questionnaire were respectively used to investigate QOL and marital quality of patients before surgery and at six months after surgery.Results:After surgery,the scores of QOL in subgroups of intervention group were statistically significantly lower than those in subgroups of control group(P<0.05);in control group,the score of QOL after surgery in total hysterectomy subgroup was statistically significantly higher than that before surgery,while in subtotal hysterectomy subgroup,hysteromyomectomy subgroup,and vascular intervention subgroup,the scores of QOL after surgery were statistically significantly lower than those before surgery(P<0.05).After surgery,the scores of marital quality in subgroups of intervention group were statistically significantly higher than those in subgroups of control group(P<0.05);in control group,the score of marital quality after surgery in total hysterectomy subgroup was statistically significantly lower than that before surgery,while in subtotal hysterectomy subgroup,hysteromyomectomy subgroup,and vascular intervention subgroup,the scores of marital quality after surgery were statistically significantly higher than those before surgery(P<0.05).Conclusion:QOL and marital quality of patients with uterine fibroid after total hysterectomy decrease significantly,psychological intervention has important impact on improving postoperative QOL and marital quality for patients with uterine fibroid.
