| 摘 要： | 目的：探讨在腰椎峡部裂的X线诊断中,＂裂隙征＂在腰椎正、侧、斜位X线片中的显示率及其诊断价值。方法：对58例诊断L5双侧峡部裂研究对象拍摄立位腰椎正、侧、双斜位X线片,分别确定L5双侧峡部裂裂隙的阳性显示率、裂隙的宽度及走形方向。9例加做腰椎CT并行二、三维重建显示裂隙的宽度和走向。分别对3个体位X线片的＂裂隙征＂显示率作配对卡方检验并分析其诊断价值。结果：在正位片上L5峡部裂隙的显示率为13.8%（8/58）,称为＂裂隙征＂,它表现为重叠的X线间接征像。侧位片L5峡部裂隙的显示率为74.1%（43/58）,斜位片L5峡部裂隙的双侧和单侧总显示率为81.0%（47/58）,两者比较差异无统计学意义（P（0.05）。将后2个体位的裂隙显示称为＂裂隙征＂。9例腰椎CT检查证实腰椎峡部裂诊断并同时显示裂隙的方向,其与冠状面或与矢状面形成不同夹角。结论：腰椎峡部裂的裂隙在正位片上显示率很低,并且是重叠的间接X线征像。在侧位和斜位片上,裂隙的阳性显示率高,是直观清晰的直接X线征像,也是腰椎峡部裂诊断的第一要素。因此,腰椎峡部裂的裂隙在X线侧位片和斜位片的直观清楚显示称为＂裂隙征＂更为准确、恰当。如在体检中X线侧位和斜位片上＂裂隙征＂阳性即可对腰椎峡部裂作出正确诊断。 |
