子宫肌瘤导管栓塞治疗的临床应用与放射介入医学研究

目的研究分析经导管子宫动脉栓塞治疗子宫肌瘤的临床疗效。方法采用Seldinger技术经股动脉穿刺插管,分别超选择插管至子宫动脉采用200-500μm的聚乙烯醇(PVA)颗粒进行栓塞12例,2例巨大子宫肌瘤手术切除前栓塞采用明胶海绵颗粒。结果 8例患者月经恢复正常,痛经明显缓解,继发贫血者血常规检查恢复正常,2例肌瘤消失、2例肌瘤缩小34%、4例缩小66%、2例缩小75%;2例闭经;4例栓塞后行子宫切除。结论经导管子宫动脉栓塞治疗子宫肌瘤,安全、可靠,不仅能使肌瘤缺血、萎缩,异常出血消失,改善临床症状,而且还能保留子宫,维持其正常的生理功能。

The Clinical Application and the Medical Research in Interventional Radiology of Uterine Fibroids Transcatheter Embolization Treatment

Objective To study and analyze the clinical efficacy of transeatheter uterine artery embolization for uterine leimyomas. Method Using catheterization of femoral artery by Seldinger technique, Separately, 12 cases were treated by super selective catheterization in uterine artery using polyvinyl aleohol（PVA） particles of 200-500μm.2 cases of Giant uterine myoma were embolized Before Surgical removal Using Absorbable Gelatin sponge particles. Results The menstruation was recovery to normal in 8 eases, dysmenorrhea was obviously relieved, blood routine tests of secondary anemia patients were recovery to normal, myoma disappeared in 2 eases, myoma shrunk by 34% in 2 eases, 66% in 4 cases, and 75% in 2 cases; 2 amenorrhea; 4 hysterectomy after embolism. Conclusion Transcatheter uterine artery embolization is a safe and reliable method for the treatment of uterine fibroids. It can not only cause fibroids ischemia, atrophy, abnormal bleeding disappeared and improve clinical symptoms,but also reserve uterus and maintain its normal physiological function.
