脑梗死复发部位、肌张力状况等对疗效及治疗成本的影响

 目的 研究脑梗死复发患者不同发病部位、入院肌张力、面瘫程度等对其治疗效果和治疗成本的影响.方法 选择40家医院在2006-03-07至2006-06-20治疗的脑梗死患者进行问卷调查,其中脑梗死复发治疗有效病例共130例,数据分析采用t检验或Willcoxon(两组),方差分析或Kruskal-Wallis检验(多组).结果 不同发病部位的脑梗死复发患者在治疗成本和治疗效果之间差异无统计学意义(P＞0.05);肌张力增高患者人均治疗总成本高于肌张力正常和肌力减低的患者(P＜0.05);SS-QOLS量表健康自评结果,肌张力减低患者预后明显不及肌张力正常和增高的患者(P＜0.01).面瘫神经功能缺损程度对患者治疗效果和治疗成本影响较小(P＞0.05).结论 脑梗死复发时不同临床症状对治疗成本和治疗效果具有一定影响.

Influence of clinical symptoms on the therapeutic effect and medical cost in patients with acute cerebral infarction

Objective To study the influence of clinical symptoms on the therapeutic effect and medical cost in patients with acute cerebral infarction. Methods Patients with cerebral infarction were enrolled using questionnaires in 40 hospitals of different grades in six provinces and cities between March 7, 2006 and June 20, 2006. A total of 130 effective cases were analyzed using t test or Wilcoxon （2 groups）, variance analysis or Kruskal - Wallis test （ multiple groups）. Results The average cost and therapeutic effect in different affected sites and degree of facial paralysis were of no significant difference （ P 〉 0.05 ）, but there was significant difference （ P 〈 0.05 ） in the total cost of treatment between different degrees of muscle tension. The self - assessment overall state of health based on SS - QOLS scale showed that patients with decreased muscle tension recovered more slowly than other two groups （ P 〈 0.01 ）. Conclusions Different clinical symptoms have some influence on patients with recurrent cerebral infarction.
