输尿管良性病变的X线诊断（附66例分析）

目的：分析输尿管良性病变的X线特征,以提高对输尿管良性病变的鉴别诊断水平。材料与方法：收集我院近4年来66例输尿管良性疾患病例,48例经手术、病理证实占72.7%,18例做碎石、抗炎治疗占27.3%。66例均行IVP造影。其中63例B超检查占95.5%,20例CT扫描占30.3%,26例逆行造影占39.4%,膀胱镜检8例占12.1%,穿刺1例。结果：B超可发现肾积水、结石。IVP造影了解肾功、输尿管阻塞、狭窄、结石X线特征。显影不良拟结合逆行造影更准确的了解狭窄特征。结论：输尿管良性狭窄、阻塞性疾患中结核、炎症病变范围较长,呈虫蚀样充盈缺损征,伴轻度肾积水;先天性狭窄多比较限局,管壁完整,肾积水较重;结石性阻塞因结石大小不同而积水程度不同,可伴炎性改变。

X-ray Diagnosis of Urinary Tract Benign Diseases(Report of 66 Cases)

Purpose： To analyze the X - ray features of urinary tract benign diseases. Materials and Methods： Total 66 cases with benign diseases of urinary tract from 2005 to 2009 years in our hospital were reviewed. Among them, 48 patients received operation, 72. 7% were diagnosed by pathology, and lithotripsy were performed on 18 cases. Anti -inflammatory therapy was carried out to 27.3% patients. Intravenous pyelography（IVP）was made on all patients, ultrasonography on 66 patients（ 95.5% ）, CT scanning on 20 patients （ 30. 3% ）, retrograde opacification on 26 patients （ 39. 4% ）, eystoscopy on 8patients, and biopsy on 1 patient. Results： Ultrasonography indicated hydronephrosis and renal calculus. IVP was helpful for kidney function, urinary tract obstruction and stricture, and X - ray features of the stones. All of these was more evident when combined with retrograde opacification. Conclusion： Benign stricture of urinary tract have manifestations of obstructive tuberculosis, prolonged inflammation, moth - eaten changes and filling defect, and even accompanied hydronephrosis. Congenital stricture is characterized with well tract wall and obvious hydronephrosis. Stone stricture is different according to the stone size.
