老年病房尿路感染病原菌的分布及耐药性分析

目的研究老年人尿路感染的常见病原菌及其对常用抗生素的耐药性,以指导临床用药。方法用法国生物梅里埃公司的Vitek-60型微生物分析仪、GNI＋鉴定卡进行菌株鉴定;药敏试验采用纸片扩散（K-B）法,对458例尿路感染的老年患者标本进行细菌培养并检测其对多种抗生素的耐药性。结果共检出病原菌651株,其中革兰阴性杆菌421株,占64.7%;革兰阳性球菌198株,占30.4%;真菌32株,占4.9%.细菌与真菌同时感染的有18例;两种细菌混合感染的有32例。分离率依次为：大肠埃希菌286株,占43.9%;肺炎克雷伯菌68株,占10.4%;阴沟肠杆菌22株,占3.4%;铜绿假单胞菌18株,占2.8%.肠球菌属细菌131株,占20.1%;凝固酶阴性葡萄球菌53株,占8.1%;金黄色葡萄球菌14株,占2.2%.结论老年人尿路感染病原菌耐药严重,持续耐药监测对抗生素的合理应用、提高疗效、减缓耐药菌株的发生与发展具有重要的意义。
