老年慢性阻塞性肺疾病患者应用氨溴索诱导痰的安全性探讨

目的评价雾化吸入氨溴索（沐舒坦）在老年慢性阻塞性肺疾病（COPD）患者中应用的安全性。方法 40例老年COPD患者随机分成A组和B组,各20例。A组用沐舒坦2m l＋3%高渗盐水2m、lB组用3%高渗盐水4m l分别进行压缩雾化诱导痰。比较2组呼气峰流速（PEF）、心率（HR）、呼吸频率（RR）、ST-T段距基线位置、血氧饱和度（SpO2）水平及不良反应发生率、成功率、痰液体积、痰液细胞总数、中性粒细胞百分比、痰液合格率。结果 A组诱导痰前后PEF、RR、HR、ST-T段位置、SpO2比较差异无统计学意义（P〉0.05）;B组诱导痰前后上述指标比较差异有统计学意义（P〈0.05）;且A组诱导痰后上述指标优于B组,差异有统计学意义（P〈0.05）。A组不良反应发生率、成功率、中性粒细胞比例及痰液合格率均高于B组,痰液体积大于B组,细胞总数多于B组,差异均有统计学意义（P〈0.05）。结论沐舒坦诱导痰对于老年COPD患者是一种安全的治疗方法。

The security analysis of inducing sputum with ambroxol in chronic obstructive pulmonary disease

Objective To establish the security of inducing sputum with ambroxol（mucosolvan） m chromc obstructive pulmonary disease（COPD）. Methods 40 cases patients with COPD were randomly divided into two groups, each of 20 cases. Group A used mucosolvan 2ml＋3% saline 2ml, group B used 3% hypertonic saline 4ml induced sputum. Compared peak expiratory flow （ PEF）, heart rate （ HR）, respiratory rate （ RR）, ST-T segment from the baseline position, oxygen saturation （ SpO2 ） and incidence of side effects, success rate, sputmn fluid volume, the total number of sputum cell, neutrophil percentage passing rate of sputum of two groups. Results Before and after inducing sputum,the levels of PEF, RR, HR,ST-T segment po- sition, SpO2 of group A were no significant difference（ P 〉 0.05 ） ; Compared the above indicators of group B （ P 〈 0.05 ） ; And the above indicators of group A were better than that of group B （P 〈 0.05 ）. The adverse reaction rate, the success rate and sputum neutrophil percentage passing rate of group A were higher than that of group B,sputum fluid volume wass greater than that of group B, the total number of sputum cell of group A were more than that of group B （ P 〈 O. 05 ）. Conclusion Muco- solvan inducing sputum in elderly COPD is a safe treatment method.
