电视胸腔镜下胸膜固定术在治疗恶性胸腔积液中的价值

目的 探讨电视胸腔镜下滑石粉喷洒胸膜固定术在恶性胸腔积液中的治疗价值.方法 恶性胸腔积液患者按就诊时间和治疗方式不同分为胸腔镜组和传统置管组,胸腔镜组在电视胸腔镜下处理积液和胸膜后喷洒滑石粉,传统置管组经胸腔闭式引流后向胸膜腔内灌注滑石粉胶浆,比较两组疗效和不良反应.结果 胸腔镜组有效率（96%）明显高于传统置管组（73%）（P＜0.05）,且住院时间、引流时间短于传统置管组,胸腔镜组有术后复张性肺水肿、肺漏气、阻塞性肺不张各1例.结论 电视胸腔镜下滑石粉喷洒胸膜固定术是控制恶性胸腔积液的有效微创方式,值得推广.

The therapeutic value of thoracoscopic pleurodesis intreating malignant pleural effusions

Objective To study the therapeutic value of thoracoscopic talc spraying pleurodesis in malignant pleural effu- sion. Methods Malignant pleural effusion patients were divided into thoracoscopic treatment group and the traditional catheter group according to different time and treatment. Thoracoscopy group patients were treated pleural and effusion with thoracoscopic, then spraying talc; conventional catheter group after pleural cavity closed drainage, perfus talc glue into pleural cavity. Compare the efficacy and adverse reactions. Results Thoracoscopic group efficiency （96%） was significantly higher than conventional cath- eter group（73% ）（P 〈 0. 05）, and hospitalization time, drainage time were shorter than conventional catheter group. Conclusion Thoracoscopic talc spraying pleurodesis sould be an effective manner in control of malignant pteural effusion, worthy promotion.
