地佐辛联合舒芬太尼用于剖宫产术后自控静脉镇痛效果观察

目的 观察地佐辛联合舒芬太尼用于剖宫产术后自控静脉镇痛（PCIA） 的效果.方法 选择ASA I～II级择期行剖宫产术的患者96例,采用持续硬膜外麻醉,术后行PCIA.按照PCIA方案的不同,随机分为地佐辛组（D组,n=32） 、舒芬太尼组（S组,n=32）、地佐辛联合舒芬太尼组（DS组,n=32）.观察3组患者术后2、8、12、24h、48h视觉模拟评分（ VAS）、镇静程度评分（ Ramsay）、48h内患者自控镇痛按压次数、术后肛门排气时间以及不良反应的情况.结果 3组配方均能为患者提供良好的术后镇痛,3组患者总的按压次数和术后肛门排气时间无统计学意义.D组患者VAS评分及Ramsay评分均显著高于S组和DS组（P＜0.05）.DS组患者和S组患者VAS评分和Ramsay评分比较,差异无统计学意义,S组不良反应发生更多.结论 地佐辛联合舒芬太尼用于剖宫产术后镇痛,镇痛效果确切可靠且不良反应少,可用于临床PCIA.

Analgesic effect of dezocine combining with sufentanil on patient-controlled intravenous analgesia in patients undergoing cesarean section

Objective To evaluate the analgesic effect of dezocine combining with sufentanil on patient-controlled intrave- nous analgesia（PCIA） in patients undergoing cesarean section. Methods 96 patients undergoing cesarean section were random- ized to receive PCIA treatment with dezocine （D group, n =32）, sufentanil（ S group, n = 32）, and dezocine combining sufentanil （DS group, n =32）. Vas, Ramsay scores, pressing times of PCA equipment, side effects and anal exhaust time were recorded and compared. Results There was no difference in the pressing times of PCA equipment and anal exhaust time among three groups. Compared with S group and DS group, the VAS and Ramsay scores in D group was higher （P 〈 0. 05 ）, and there was no difference in VAS and Ramsay scores between S group and DS group（ P 〉0. 05）. The occurence rate of side effect in S group was higher than D group and DS group （P 〈 0. 05 ）. Conclusion Decozine combining with sufentanyl exhibits better analgesic effect with less side effects, and may reprensent a better PCIA stragety in patients undergoing cesarean section.
