IPC预防ICU重症患者深静脉血栓的成本效益分析

目的 探讨使用间歇充气加压治疗（intermittent pneumatic compression,IPC）预防ICU重症患者发生静脉血栓栓塞症（venous thromboembolism,VTE）的成本效益.方法 收集相关数据：未使用IPC预防的重症患者VTE的发生情况,使用IPC预防VTE的效果及成本,检查及治疗VTE的成本.分析IPC预防重症患者发生VTE的成本效益.结果 在未使用IPC预防VTE的重症患者中,一旦发生VTE,人均增加的检查及治疗成本为$2754.6;在使用IPC的患者中,VTE的发生率显著降低,其发生VTE后人均增加的检查及治疗成本加上IPC使用成本合计为$1081.1;使用IPC预防VTE的人均效益为$1673.5.IPC预防VTE的效益成本比为3.9∶1.结论 使用IPC不但可以预防重症患者VTE的发生,还可以减少重症患者的住院费用.

Cost benefit analysis of VTE prophylaxis with IPC in critically ill patients in ICU

Objective To determine the costs and savings of IPC for venous thromboembolism prophylaxis in critically ill patients in ICU. Methods VTE events after ICU admission in the absence of prophylaxis, efficacy of prophylaxis and costs of prophylaxis with IPC had been obtained and the cost benefit were analyzed for VTE prophylaxis with IPC. Results In the absence of prophylaxis, the increased cost of investigating and treating VTE was 2754. 6 for per critically ill patient. The cost of IPC and the increased cost of investigating and treating the reduced numbers of VTE was 1081.1 for per critically ill patient who was re- ceived VTE prophylaxis with IPC. This corresponded to a saving of 1673.5 for per critically ill patient. The benefit-cost ratio was 3.9 ： 1. Results VTE prophylaxis with IPC in critically ill patients in ICU might not only prevented VTE, but also saved money.
