抗感染用喹诺酮类药分级使用与用药频度分析

目的：了解上海市闵行区喹诺酮类药的使用现状及特点,为贯彻落实抗菌药物限制使用制度提供参考.方法：统计该区15家医院2012年喹诺酮类药的集中购药数据,采用金额排序、用药频度（DDDs）排序和限定日费用（DDC）法,对不同级别医院的具体用药品种进行分析.结果：2012年分级管理目录内品种购药金额占全部喹诺酮类药的95.87％,一级医院口服剂型购药金额略高于注射剂型,占55.90％;二、三级医院注射剂型高于口服剂型,分别占74.29％和63.91％;三个不同级别医院DDDs排序列第1位的均是左氧氟沙星（口服）,一级医院DDDs最高为47.61;一、二、三级医院口服与注射药物的DDC分别为1.56元/20.92元、4.49元/66.94元、5.06元/58.02元.结论：一、二、三级医院使用限制使用级药物的DDDs分别为10.53、40.69、20.56,表明二级医院选用喹诺酮类药居多.

Classified Use and DDDs of the Anti-infective Drug Quinolones

OBEJECTIVE：To investigate the status quo and characteristics of clinical use of quinolones in Minhang district of Shanghai so as to provide reference for implementing the system of restricted use of antibiotics.METHODS：The centralized purchasing data of quinolones in 15 hospitals in 2012 were collected and analyzed with regard to ordering of consumption sum,DDDs and DDC as well as the drug varieties used in hospitals of different levels.RESULTS：In 2012,the consumption sum of the quinolones covered by hierarchical management list in 2012 accounted for 95.87% ; the consumption sum of the oral quinolones in first level hospital accounted for 55.90%,which was slightly higher than that of the quinolones for injection; however,in second level and third level hospitals,quinolones for injection accounted for 74.29% and 63.91% respectively,which were higher than their oral counterparts.Levofloxacin （oral dosage form） took the lead on the list of DDDs in hospitals of three different levels,especially in level one hospital which stood at 47.61; the DDDs of oral quinolones vs.quinolones for injection in hospitals of level one,two and three stood at 1.56 yuan vs.20.9 yuan,4.49 yuan vs.66.94 yuan and 5.06 yuan vs.58.02 yuan,respectively.CONCLUSIONS：The DDDs of the quinolones with restricted use in hospitals of level one,two and three were 10.53,40.69 and 20.56 respectively,indicating more frequent use of quinolones in hospitals of level two.
