CD64表达在手足口病继发细菌感染患儿中的诊断价值

目的 探讨中性粒细胞CD64的表达在手足口病患者并发细菌感染中的诊断价值.方法 选择手足口病并确诊为并发细菌感染的住院患儿38例,无细菌感染患儿52例,应用流式细胞术测定中性粒细胞CD64,同时测外周血C-反应蛋白(CRP)、白细胞及分类进行比较.结果 并发感染组治疗前CD64水平为(14.51±6.07)MFI,明显高于治疗后组的(10.6±3.23) MFI及未感染治疗前组(9.55±3.48)MFI,差异有统计学意义(t=5.7、3.4,P＜0.05)；根据ROC曲线,以CD64≥11.35MFI为阳性标准,其诊断并发细菌感染的敏感度、特异度和准确度分别为82.25％、78.91％及81.32％.结论 中性粒细胞CD64的表达可作为手足口病并发细菌的检测指标.

Clinical value of CD64 expression in diagnosis of bacterial infections in children with hand foot and mouth diseases

OBJECTIVE To study the clinical value of expression of peripheral blood neutrophil CD64 in diagnosis of bacterial infections in the children with hand foot and mouth diseases.METHODS A total of 38 hospitalized children with hand foot and mouth disease complicating bacterial infection and 52 children without bacterial infections were enrolled in the study,the determination of CD64,C-reactive protein(CRP),white blood cell(WBC) count and percentage of neutrophil of each patient was performed by using flow cytometry.RESULTS The expression of CD64 in the patients with bacterial infections before the treatment was(14.51±6.07)MFI,which was significantly higher than(10.6±3.23) MFI of the group at recovery stage and(9.55±3.48)MFI of the patients without infections before the treatment,the difference was statistically significant(t=5.7 and 3.4,P<0.05).These markers were considered positive if CD64≥11.35 MFI.According to ROC curve,the sensitivity,specificity and accuracy of the diagnosis of bacterial infections were 82.25%,78.91% and 81.32%,respectively.CONCLUSION The expression of peripheral blood neutrophil CD64 can be used as an indicator in the diagnosis of bacterial infections in the patients with hand,foot and mouth diseases.
