一次性纸巾干手物品应用于免洗外科手消毒法中的效果与成本分析

目的 研究一次性纸巾干手物品在免洗外科手消毒法中的应用效果与成本,为临床选择安全可靠、便捷经济的干手方法提供科学依据.方法 选取2011年1-10月医院经过手卫生规范培训的外科手术人员(主刀医师、一助医师、洗手护士)400名,随机分为试验组和对照组,均对指甲进行修剪并彻底清洁,使用皮肤清洗剂按六步洗手法清洗双手及前臂至肘上10 cm处的污垢,流动水冲净并擦干,进行免洗外科手消毒；两组分别采用一次性纸巾和灭菌毛巾干手,并对手指进行采样监测.结果 两组免洗外科手消毒后手指培养细菌菌落总数≤5CFU/cm2,且未检出金黄色葡萄球菌、大肠埃希菌、铜绿假单胞菌等,达到国家卫生部2002年版《消毒技术规范》标准；使用灭菌毛巾干手的月支出约为5400元,使用一次性纸巾干手的月支出约为1440元.结论 一次性纸巾干手法可节约手消毒成本,简化操作流程,提高工作效率.

Effect and cost analysis of disposable hand towels applied for dry surgical hand disinfection

OBJECTIVE To analyze the cost-effect of disposable hand towels applied for dry surgical hand disinfection so as to provide scientific basis for the selection of a reliable,safe,convenient and economical hand-drying method.METHODS From Jan 2011 to Oct2011,we recruited 400 surgical department staff(including the surgeon,physicians assistants,and the nurses) and randomly divided into two groups.All the staff cut the fingernail and thoroughly cleaned and used six-step hand-washing method to wash their hands and forearms to the elbow on the 10 cm,rinsed with running water and dried,and then had dry surgical hand disinfection,the hands were dried by respectively using disposable hand towels and sterilized towels,the fingers were sampled.RESULTS The total bacterial colony counts of the two groups were less than 5 CFU/cm2 after the drug surgical hand-washing,and Staphylococcus aureus,Escherichia coli,and Pseudomonas aeruginosa were not detected,which met the requirements of Technical Standard for Disinfection 2002 edition,the monthly cost of the sterile towels was 5400 yuan,disposable hand towels 1440 yuan.CONCLUSION Disposable hand towels save the hospital cost and simplify the operation procedure and improve the efficiency as well.
