糖尿病并发尿路感染的感染因素分析

目的 探讨糖尿病患者并发尿路感染的影响因素,为临床预防及治疗提供依据.方法 选取医院2010年2月-2011年10月收治的60例糖尿病并发尿路感染患者病例作为观察组,与同期120例无尿路感染糖尿病作为对照组,比较两组一般资料、糖化血红蛋白(HbAc1)、并发症、治疗,并进行对比分析.结果 两组患者影响因素对比,观察组60例患者HbAc1值、并发症、住院时间、预防应用抗菌药物比例较对照组显著,观察组各项因素高于对照组,比较差异有统计学意义；60例泌尿系感染患者共分离病原菌56株,其中革兰阴性菌36株占64.29％,革兰阳性菌15株占26.79％,真菌5株占8.92％,观察组尿路感染60例,治愈46例占76.67％,好转12例占20％,无效2例占3.33％,无效患者有慢性并发症,病程＞5年.结论 糖尿病并发尿路感染与空腹及餐后2h血糖高、慢性并发症等因素有紧密关系,控制血糖、预防相关并发症、合理使用抗菌药物对防治本病有重要作用.

Influencing factors for urinary tract infections in diabete patients

OBJECTIVE To investigate the diabete patients complicated with urinary tract infections and the influencing factors,provide evidence for clinical prevention and treatment.METHODS A total of 60 cases of diabete patients complicated with urinary tract infections who were treated in the hospital from Feb 2010 to Oct 2011 were selected as the observation group,and 120 patients without urinary tract infections were selected at the same period as the control group.The general data,glycosylated hemoglobin(HbAc1),complications,and treatment of the two groups of patients were statistically analyzed.RESULTS As compared with the influencing factors,the HbAc1 value,complications,hospitalization duration,and the prophylactic use of antibiotics in proportion of the 60 patients in the observation group were more significant than those of the patients in the control group,the various factors of the observation group were higher than those of the control group,the difference was statistically significant.Totally 56 strains of pathogens were isolated from 60 patients with urinary tract infections,including 36(64.29%) strains of gram-negative bacteria,15(26.79%) strains of gram-positive bacteria,and 5(8.92%) strains of fungi.Of 60 patients with the urinary tract infections in the observation group,there were 46(76.67%) patients cured,12 patients improved,and 2 cases invalid.The patients of the invalid treatment were complicated with the chronic complications with the disease course more than 5 years.CONCLUSION The diabetes complicated with urinary infections are closely related to the elevated fasting and 2 h-postprandial and blood glucose.The control of blood glucose,prevention of the related complications,and the reasonable use of antibiotics play important roles in the prevention of this disease.
