恶性血液病患者并发肛周感染的临床观察

目的 探讨恶性血液病患者并发肛周感染的临床相关因素.方法 对医院2008年2月-2011年3月收治的恶性血液病患者并发肛周感染的相关因素进行分析,并对其治愈时间进行比较.结果 化疗未缓解者肛周感染24例,感染率28.57％;血液病类型中多发性骨髓瘤并发肛周感染5例,占20.83％；慢性粒细胞白血病并发肛周感染8例,占17.78％；年龄因素中＞50岁患者肛周感染14例,占14.43％;粒细胞绝对值＜0.5肛周感染7例,占6.86％,化疗效果、血液病类型、年龄、粒细胞绝对值是并发肛周感染的重要影响因素；手术治疗组15例,治愈平均17.5d,发热平均7.5d.保守治疗组13例,治愈平均28.6d,发热平均12.6d,两组比较,差异有统计学意义(P＜0.01).结论 恶性血液病患者并发肛周感染与年龄、化疗、粒细胞绝对值、血液病类型等因素有重要关系,对于严重感染患者及时采取手术切开引流有重要作用.

Clinical observation on malignant blood disease patients complicated with perianal tissue infections

OBJECTIVE To explore the related factors for the perianal tissue infections in the patients with hematologic malignancies.METHODS From Feb 2008 to Mar 2011,the related factors for the perianal tissue infections in the patients with malignant hematologic diseases were analyzed.The healing time was compared.RESULTS The perianal tissue infections occurred in 24 patients undergoing chemotherapy without remission,the infection rate was 28.57%;there were 5(20.83%)cases with multiple myeloma infections among the blood disease types;there were 8(17.78%) cases of chronic myeloid leukaemia patients with infections;there were 14(14.43%) infections cases aged more than 50 years;there were 7(6.86%) cases of patients with the absolute granulocyte value less than 0.5,the effect of chemotherapy,blood disease type,age,and absolute granulocyte value were the main influencing factors for the perianal tissue infections;there were 15 patients undergoing surgical treatment with the median cure time of 17.5 d and median fever of 7.5 d,there were 13 patients receiving the conservative treatment with the median cure time of 28.6 d and the median fever time of 12.6 d,as compared with the two groups,the differences were statistically significant P<0.01).CONCLUSION The perianal tissue infections in the patients with malignant blood diseases are closely related to the age,chemotherapy,absolute granulocyte value,and the blood disease type;for the patients with severe infections,it is necessary and important to perform the surgical incision drainage in a timely manner.
