医院感染相关医疗纠纷的原因分析

目的 总结国内医院感染相关医疗纠纷病例的感染因素、纠纷发生原因,为降低医院感染相关性医疗纠纷的发生和医院感染控制提供依据.方法 以医院感染为第1检索词、医疗纠纷为第2检索词,对2000年1月-2011年6月国内公开发表的中文文献统一纳入和排除标准,所得资料进行总结分析.结果 发生医疗纠纷共501起,与医院感染相关的医疗纠纷99例,占19.76％；其中术后切口感染占29.29％、术后并发症占23.23％、住院费用增加占15.15％、沟通不良占11.11％、抗菌药物使用不合理占4.04％,为引发医疗纠纷的主要原因.结论 医院应进一步加强医院感染相关医疗纠纷的防范意识,从重点环节人手,防止医院感染的发生,确保医疗安全.

Meta-analysis of causes of nosocomial infection-related medical malpractice

OBJECTIVE To summarize the causes of hospital infection-associated medical dispute in China so as to reduce the nosocomial infection-associated medical dispute and provide bases for the control of nosocomial infections.METHODS By using the hospital infection as the first search term,medical malpractice and the second search terms,Chinese literature published from Jan 2000 to Jun 2011 were uniformly included and excluded according to the criteria.RESULTS There were totally 501 medical malpractice including 99(19.76%) cases of hospital infection-related medical disputes;the main causes of medical disputes mainly included the postoperative wound infections(29.29%),postoperative complications(23.23%),the increase in medical costs(15.15%),poor communication(11.11%),and inappropriate use of antimicrobial drugs(4.04%).CONCLUSION It is necessary to further strengthen the awareness of prevention of nosocomial infection-associated medical dispute and start with the key links so as to prevent nosocomial infections and ensure the medical safety.
