微血管密度及血管内皮生长因子、p53表达与非小细胞肺癌预后的相关性研究

摘    要:目的探讨微血管密度(microvessel density,MVD)和血管内皮细胞生长因子(vascular endothelial growth fac-tor,VEGF)、p53表达与非小细胞肺癌(non small cell lung cancer,NSCLC)预后的关系。方法应用免疫组化S-P法,检测47例非小细胞肺癌VEGF和p53表达,并用抗FVIII因子抗体标记瘤组织血管内皮细胞,计算MVD,并结合临床资料分析。结果p53、VEGF总阳性表达率分别为53.2%(25/47)和38.3%(18/47)。VEGF表达阳性者和VEGF表达阴性者3年生存率分别为29.4%和73.3%,差异有显著性(P<0.05)。p53表达阳性者和p53表达阴性者3年生存率分别为38.5%和81.0%,差异有显著性(P<0.05)。MVD>33者和MVD<33者3年生存率分别为38.1%和73.1%,差异有显著性(P<0.05)。结论在非小细胞肺癌血管生成过程中,VEGF、p53基因表达上调在调控肿瘤血管形成方面起重要促进作用。MVD值增大,非小细胞肺癌易发生转移,3年生存率越低;VEGF、p53阳性表达率上调,MVD值增大,预后亦越差。

Abstract:Objective To study the relationship between microvessel density (MVD), vascular endothelial growth factors (VEGF) , the expression of p53 and the prognosis of non-small cell lung cancer (NSCLC). Methods A total of 47 specimens of NSCLC were tested by immunohistochemical method (S-P method) for MVD and expressions of VEGF and p53. Results The positive rates of p53 and VEGF were 53.2% (25/47) and 38.3% (18/47). The 3-year survival rates of patients with positive expressions of VEGF and p53 were 29.4% and 38.5%, which were significantly lower than those who negatively expressed VEGF and p53. The 3-year survival rate was significantly lower in MVD-high patients than those with MVD-low patients (38.1% vs 73.1%, P<0.05). Conclusions p53 gene and VEGF might play an important role in promoting tumor angiogenesis. These findings support that the higher MVD is related to lower 3-year survival rate, and metastasis is more likely to occur. The positive expressions of p53 and VEGF indicate higher index of MVD and poor prognosis in NSCLC.
