腹腔镜与开腹直肠癌根治术围手术期并发症及预后比较

目的：比较分析腹腔镜与开腹直肠癌根治术围手术期并发症发生率及预后。方法对安徽医科大学第一附属医院2009年2月至2011年5月施行腹腔镜直肠癌根治术(腹腔镜组,72例)和开腹直肠癌根治术(开腹组,125例)患者的围手术期并发症发生率及预后进行比较分析。结果腹腔镜组与开腹组的总并发症发生率、术中并发症及3年生存率差异无统计学意义(P>0．05)；腹腔镜组术后并发症发生率低于开腹组,差异有统计学意义(P<0．05)。结论腹腔镜和开腹直肠癌根治术均可以达到较好远期预后,腹腔镜手术不增加术中并发症风险,术后并发症总发生率低于开腹手术。

Comparison of perioperative complication and prognosis following laparoscopic and open rectal cancer resection

Objective To compare the perioperative complications and prognosis rate following laparoscopic and open rectal cancer resection. Methods From February 2009 to May 2014, 72 rectal cancer patients underwent laparoscopic surgery(LP group) and 125 open surgery( OP group) in the hospital were recruited in the comparative study. The perioperative complications and the prognosis were compared between the two groups. Results There was no statistical difference in the overall complication rate, intraoperative complication rate and three years of survival between LP and OP group(P>0. 05). The postoperative complication rate in LP group was lower than that in OP group with significant difference(P<0. 05). Conclusion Both laparoscopic and open surgery of radical rectal resection can achieve a long-term prognosis. The laparoscopic surgery does not increase the risk of intraoperative complications while the postoperative complication rate is lower in laparoscopic surgery than in open surgery.
