胸腔镜术治疗自发性气胸的进一步探讨

该院自１９９５年５月～１９９７年７月应用胸腔镜术治疗各种类型的自发性气胸２４例。除２例因早期经验不足中转开胸外，２２例均在胸腔镜下完成，疗效满意，无并发症，无手术死亡。结合手术经验，该文对自发性气胸的手术适应证，手术时机，手术方法的选择进行了进一步探讨。认为内镜持针器持针线行肺大泡缝扎切除的手术方法和干纱布摩擦壁层胸膜固定术防止复发的方法值得推广。

THE FURTHER STUDY OF THORACOSCOPY FOR TREATMENT OF SPONTANEOUS PNEUMOTHORAX

From May 1995 to July 1997,twenty-four patients suffering from various spontaneous pneumothorax were performed using video-assisted thoracoscopic surgery(VATS) in our hospital.Among them,16 cases were simple spontaneous pneumothorax,one case was tension spontaneous pneumothorax,four cases combined with hemothorax,two cases with pulmonary incomplete expansion,one case with long-term chronic pulmonary atelectasis.22 cases underwent VATS,except two cases who were turned to thoracotomy because of lackness of experience in the early time.We gain the satisfactory result.No death and complication.Combined issues with experience of operation,we have taken the further study for the operative indication,operative chance and the selection of operative methods of spontaneous pneumothorax.We consider it is worth spreading to the operative methods of using endoscopic holder with thread to suture the bullae of lung and then removing them and preventing recurrence method of pleurodesis rubbing parietal pleura with dry gauze.
