扶贫白内障手术效果影响因素分析

目的:探讨贫困地区和城市地区人群白内障手术效果差异,分析影响因素。方法:2007年1月~2007年10月期间我院实施的2835例白内障手术,其中800例扶贫白内障手术为扶贫组,城市地区植入相同人工晶体白内障手术688例为城区组,对两组病例进行临床资料分析,得出手术并发症、术后复明效果的差异,分析其影响因素。结果:(1)扶贫组超声乳化率为72.75%,城区组为94.33%,P<0.01;(2)扶贫组并发症发生率14.38%,城区组为9.59%,p<0.01;(3)扶贫组脱残率76.25%,城区组为91.14%,p<0.01具有差异性;扶贫组术后脱盲率93.50%,城区组为97.67%。p<0.01具有差异性。(4)扶贫组术后复明效果影响因素,其中与角膜情况差、晶体混浊程度重,硬核白内障比例高等有关。结论:扶贫白内障手术情况复杂,复明手术效果影响因素多。贫困地区防盲、治盲工作需要加强,特别是常见眼病的及时诊治。

Analysis of Effects of Cataract Surgery in Needy Region

Objective:To evaluate the effects of cataract extraction in needy region compared to urban districts.Methods:1488 cases treated by cataract surgery were used in this study,composed of 800 cases in needy region and 688 cases in urban districts.Vision rehabilitation and complication were analyzed and compared between two groups.Results:(1)Rate of phacoemulsification was 72.75% in needy region group and 94.33% in urban districts group(P<0.01).(2)Incidence of complication was 14.38% in needy region group and 9.59% in urban districts group(P<0.01).(3)Rate of vision rehabilitation was 93.50% in needy region group and 97.67% in urban districts group(P<0.01).(4)Vision rehabilitation in needy region group was correlated with the poor cornea state,severe opacity of lens and great proportion of harder nuclei.Conclusion:Cataracts in needy region are diverse and many influencing factors contribute to the effects of cataract surgery.Prevention of blindness in needy region should be strengthened,especially effective diagnosis and treatment of common ophthalmocac.
