二维钛网数字化塑形技术在颅骨修补术中的临床应用

摘    要:目的探讨二维钛网数字化塑形技术在颅骨修补术中的临床应用价值。方法观察组将患者薄层CT的DICOM数据进行三维重建后,建立颅骨缺损的原始石膏模型和修复体的数学模型,将数学模型传输到数字化成型设备上,对二维钛网进行无模压制、成型。术前将塑形钛网与患者缺损区进行嵌对后进行颅骨修补。随机抽取以往采用传统三维钛网颅骨修补术的21例患者作为对照组。比较分析两组的手术时间、钛钉使用数、恢复时间、积液感染率、引流量、术后塑形满意度等。结果观察组平均手术时间为(1.7±0.4)h,钛钉平均使用数量为(15±0.7)枚,术后平均恢复时间为(7.2±1.2)d,积液发生率为4.85%,感染发生率为0,切口引流量(70±8)ml,外形满意率为100%;对照组分别为(2.7±0.6)h,(21±0.8)枚,(910.3±1.4)d,29%,12%,(180±13)ml,67%。两组手术时间、钛钉数、恢复时间、积液感染率、引流量、外形满意度相比,P〈0.05。结论应用数字化塑形二维钛网修补颅骨缺损操作简便,手术时间短,恢复时间短,费用较少,术后并发症少,缺损区外观修复满意,具有广泛应用价值。

Abstract:Objective To explore the clinical value of the digitization forming technique used in 2D-titanium mesh skull patch surgery.Methods After 3D-reconstrution of the CT DICOM data,original plaster model of skull defects and mathematical model of restoration were established,2D-titanium mesh was formed according to the mathematical model through digital molding devices.Titanium mesh was embedded on the defect before skull impairment patch surgery.Surgical time,titanium nail number,restores time,satisfaction for the shape and complications were observed,and compared with the group which used traditional 3D-titanium mesh skull impairment patch surgery.Results Compared with the control group,the surgical time and titanium nail number reduced and they had better postoperative recovery.There were no infections of incisional wound and subcutaneous fluid,etc.The shape of the head was symmetrical and the forming was more satisfied.Conclusion The digitization forming technique in 2D-titanium mesh for repairing the skull is convenient,inexpensive and quick,and has satisfactory shape.It is worthy of widespread application.
