| 摘 要： | 目的进一步提高儿童脓毒血症的早期诊断率,探讨一种快速可靠的方法。方法将武汉大学人民医院收治的124例儿童脓毒血症患者分为观察组（56例）及对照组（68例）,两组进行降钙素原（PCT）测定,并与超敏C-反应蛋白（hs-CRP）、白细胞计数（WBC计数）进行比较。结果 PCT检测结果与血培养结果相比,差异无统计学意义（χ2=2.77,P〉0.05）。以PCT≥2 ng/ml和hs-CRP≥10 mg/L为临界值,观察组PCT及hs-CRP阳性率均明显高于对照组,且两组比较,差异均有统计学意义（P〈0.01）。白细胞计数两组比较,差异无统计学意义（χ2=3.56,P〉0.05）。PCT诊断脓毒血症的灵敏度、特异度,阳性预测值,阴性预测值,准确性,约登指数均明显优于WBC计数,与hs-CRP相比,PCT的特异性更高。结论 PCT检测作为实验室指标,有助于早期诊断儿童脓毒血症,判断病情、评价疗效和估计预后。
