摘    要:目的:探讨血清、脑脊液神经元特异性烯醇化酶(NSE)与颅脑损伤程度及其预后的关系.方法:用ELISA法测定了208例急性颅脑损伤患者的血清、脑脊液NSE含量,结合昏迷和预后分级进行比较分析.结果:颅脑外伤患者血清、脑脊液NSE浓度明显高于对照组;重型、中型脑外伤血清、脑脊液NSE水平高于轻型组;硬膜外血肿组、硬膜下血肿组、脑挫裂伤组的血清、脑脊液NSE明显低于合并组;死亡和植物生存组、残疾组的血清、脑脊液NSE水平高于恢复良好组;重型颅脑损伤患者血清、脑脊液NSE浓度,随病程变化逐渐降低,差异有统计学意义.结论:早期颅脑损伤后,血清、脑脊液NSE水平在一定程度上反映颅脑损伤程度,也是预后评估的重要指标之一.

Abstract:Objective:To understand the relationship between change of neuron specific enolase(NSE) in serum and cerebrospinal fluid(CSF) and traumatic brain injury.Methods:208 cases of acute traumatic brain injury were assigned to the dead and plant survival group(GOS 1~2,n=28),disability group(GOS 3~4,n=72) and good recovery group(GOS 5,n=108),and 30 healthy subjects serve as control group.The serum and CSF level of NSE was detected by enzyme-linked immunosorbnent assay.The relationship between change of NSE in CSF and traumatic brain injury was assessed.Results:Serum and CSF levels of NSE were higher in patients with brain injury than those in controls,and these level were higher in patients with severe brain injury than that in normal group,meanwhile,they were higher in complex cases than in cases of single disease.Conclusions:Serum and CSF level of NSE could be a prognostic parameter in patients with traumatic brain injury.
