失效模式与效应分析在胰腺外科手术患者非计划性拔管中的应用研究

目的 探讨失效模式与效应分析在降低胰腺外科手术患者非计划性拔管发生率中的应用效果。 方法 选取2020年11月至2021年1月胰腺外科手术后留置导管的304例患者为研究对象,将2019年11月至2020年1月的胰腺外科手术患者304例设为对照组。对照组落实常规的导管护理。研究组在此基础上采用失效模式与效应分析,进行护理措施的改进。比较两组患者非计划性拔管的发生率。结果 失效模式与效应分析方法应用后,医疗护理行为、导管固定失效、肢体约束不当、舒适度差、拔管风险评估缺陷、健康教育不适宜方面得分均显著低于实施前风险值得分有显著下降(P<0.05),研究组的非计划性拔管发生率低于对照组(P<0.05)。结论 采用失效模式与效应分析的管理使胰腺外科手术患者的非计划性拔管的发生率显著下降。

Application of failure mode and effect analysis in unplanned extubations of pancreatic surgery patients

Objective: To investigate the effect of failure mode and effect analysis in reducing the incidence of unplanned extubation in pancreatic surgery patients. Methods: A total of 304 patients with indwetting catheter after pancreatic surgery from November 2020 to January 2021 were selected as the study group, and 304 patients with indwetting catheter after pancreatic surgery from November 2019 to January 2020 were selected as the control group. The control group received routine catheter care. On this basis, the study group adopted failure mode and effect analysis on the basis of routine catheter care to improve nursing measures. The incidence of unplanned extubation was compared between the two groups. Results: After the application of failure mode and effect analysis method, the risk value score was significantly decreased (P<0.05), and the incidence of unplanned extubation in the study group was lower than that in the control group (P<0.05). Conclusion: The management of applying failure mode and effect analysis can significantly reduce the incidence of unplanned extubation in pancreatic surgery patients, and improve the relevant protection knowledge of all types of personnel.
