妊娠合并子宫肌瘤的类型对妊娠并发症的影响

目的探讨妊娠合并子宫肌瘤的类型对妊娠并发症的影响。方法回顾性分析2007年8月至2010年6月在我院治疗的302例妊娠合并子宫肌瘤患者的临床资料,按肌瘤类型进行分组,对其妊娠并发症进行比较,进一步研究肌壁间肌瘤大小对妊娠并发症的影响。结果与浆膜下肌瘤组比较,肌壁间肌瘤组、粘膜下肌瘤组胎膜早破的发生率无明显增加（P〉0.05）,而早产、胎位异常、前置胎盘、产后出血的发生率显著增加（P〈0.05）;与〈2cm肌瘤组、2～5cm肌瘤组相比较,〉5cm肌瘤组胎膜早破、早产、胎位异常、前置胎盘、产后出血发生率明显增加,且产后出血量明显增多。结论 〉5cm肌壁间肌瘤、粘膜下肌瘤明显增加妊娠期、产时、产后并发症的发生率,应于孕前高度重视。

The relevance of subtype of uterine myoma and pregnancy complication

Objective To study the effect of pregnancy complicated with different type of uterine myoma on pregnancy complication.Methods 302 cases of pregnancy with uterine myoma were analysed retrospectively from August 2007 to June 2010 in our hospital.According to subtype and size of uterine myoma,pregnancy complications were compared.Results Compared with subserosal uterine myoma,the incidence rates of premature delivery,abnormal presentation,placenta previa and postpartum hemorrhage with intramural and submucosal uterine myoma were increased significantly （P〈0.05）,but it was not increased in premature rupture of membrane （P〈0.05）.Significant differences on premature rupture of membranes,premature delivery,abnormal presentation,placenta previa and postpartum hemorrhage were found between 2cm,2 ～5cm intramural myoma group and 〉5cm myoma group （P〈0.05）.Conclusion The incidence rates of pregnancy complication with 〉5cm intramural and submucosal uterine myoma were increased significantly.
