腹腔镜卵巢打孔术治疗多囊卵巢综合征22例近期疗效观察

摘    要:目的探讨电视腹腔镜下卵巢打孔术治疗多囊卵巢综合征(PCOS)合并不孕症的近期疗效。方法选择2005年12月~2009年9月我院妇科收治的确诊为PCOS的不孕症患者22例,选择在月经干净3~7天给予腹腔镜卵巢打孔术(LOD),术后定期观察月经、卵巢排卵及妊娠情况,3个月未孕者使用促排卵药物,比较手术前后血清雄激素(T)、黄体生成激素(LH)、卵泡刺激素(FSH)值的变化及月经情况的改变。结果术后血清性激素T、LH明显下降,LH/FSH比值降低,与术前相比差异有极显著意义(P〈0.01),月经均已恢复正常,排卵率81.8%,妊娠率68.2%,早期流产率12.5%。结论腹腔镜卵巢打孔术是治疗PCOS不孕症的有效治疗手段。

Abstract:Objective To observe the therapeutic effect of laparoscopic ovarian drilling (LOD) on infertile women with polycystic ovary syndrome (PCOS). Methods 22 patients with PCOS were treated by laparoscopic ovarian drilling (LOD) 3~7 days after menstruation. The menstrual pattern and ovulation changes, the pregnancy outcome were evaluated and failed pregnancy patients for three months took ovulation induction medicine. The serum concentration changes of testosterone (T),LH (luteinizing hormone), follicle stimulating hormone (FSH)serum levels and The menstrual pattern were compared after surgery. Results There was a significant reduction (P0.01) in serum testosterone and LH level, and so was the ratio of LH/FSH. The ovulation rate was 81.8% , pregnancy rate was 68.2% and early abortion rate was 12.5%. Conclusion Laparoscopic ovarian drilling is an effective therapy approach to PCOS with infertility.
