腹腔镜下卵巢成熟畸胎瘤剔除术临床疗效分析

摘    要:目的探讨腹腔镜下卵巢成熟畸胎瘤剔除术的临床疗效。方法回顾分析我院和南村医院2004年1月至2007年1月腹腔镜下卵巢成熟性畸胎瘤剔除术患者83例(腹腔镜组)和开腹手术47例(开腹组)的临床资料。结果腹腔镜组手术时间为(30.4±12.0)min,开腹组为(40.6±5.9)min,两组差异有统计学意义;腹腔镜组术中出血量为(18.5±5.7)ml,显著少于开腹组的(50.0±20.8)ml;腹腔镜组术后肛门排气时间为(20.1±3.7)h,显著短于开腹组的(30.4±8.2)h;腹腔镜组术后住院时间为(4.0±1.2)d,显著短于开腹组的(7.1±1.8)d。两组患者术后均无化学性腹膜炎发生。全部患者随访3~4年,两组均无复发。结论应用腹腔镜剔除卵巢成熟性畸胎瘤安全而有效,是治疗卵巢畸胎瘤较理想的方法之一。

Abstract:Objective To investigate the clinical effect of laparoscopic removal of ovarian mature teratoma. Methods A retrospective analysis was made on clinical data of 83 cases of ovarian mature teratoma enucleation by laparoscopy(laparo-scopic group) and 47 cases of open ovarian dermoid cyst (open group) from January 2004 to January 2007. Results There was difference in the operation time between the laparoscopic group (30.4±12.0) min and the open group(40.6±5.9)min,less intra-operative blood loss (18.5±5.7)ml than the open group (50.0±20.8)ml,the time to first defecation was significantly shorter in the laparoscopic group (20.1±3.7) hours than in the open group (30.4±8.2) hours,shorter hospitalization (4.0±1.2)d than the open group (7.1±1.8) d,P〈0.05. No one had chemical peritonitis after operation in both group. All patients were followed up 3~4 years. Each group has none relapse. Conclusion Laparoscopic removal of ovarian mature teratoma is a feasible and effective procedure,being one of ideal methods for the treatment of ovarian mature teratoma enucleation.
