改良式Kugel补片在股疝嵌顿中的应用

目的评价Modified Kugel（改良Kugel）疝修补装置在股疝嵌顿无张力修补术中的临床应用。方法采用MK修补装置,治疗26例股疝嵌顿患者,暴露腹股沟管后切开腹股沟韧带,松解嵌顿的股疝。暴露股管及游离腹膜前间隙后使用下层补片在腹膜外封闭股管入口及整个耻骨肌孔。上层补片覆盖并保护整个腹股沟管后壁。结果无术后死亡病例,手术时间60～90min;术后3天可以正常活动;平均住院天数8.5d;术后1例患者切口感染,经局部引流及换药处理后好转,未取出补片。全部患者术后随访6～36月,未见复发。结论MK疝修补装置在保护腹股沟区和股区有明显优势,术后不适感很少,活动很少受限。由于形成双重保护机制,术后复发率较低。

The modified kugel patch repair in femoral strangulated hernia

Objective To evaluate the clinical effect of the modified kugel patch repair in femoral strangulated hernia. Methods We performed operation on 26 femoral strangulated hernia patients using the modified kugel patch. Results There was no postoperative mortality. The operation time was 75 minutes （range 60～90）. The mean length of hospital day was 8.5 days. Operation complications were few and no severe. The follow-up period ranged from 6 to 36 months,and no ecurrence was observed. Conclusion The modified kugel patchs repair is new,open,minimally invasive procedures using a preperitoneal approach. They are effective,low recurrence rate.
