那格列奈与阿卡波糖在初发老年2型糖尿病中的临床应用

目的比较那格列奈与阿卡波糖在初发老年2型糖尿病患者中的有效性及依存性。方法 50例初发的老年2型糖尿病患者随机、双盲分为两组,进行为期12周临床观察,分别比较空腹血糖（FPG）、餐后2小时血糖（2hFPG）、糖化血红蛋白（HbA1c）、不良反应。结果治疗后4周、12周相比,两组空腹血糖水平无显著性意义（P〉0.05）。治疗4周、12周相比,餐后2h血糖（2hFPG）水平均显著低于治疗前（P=0.00）,两组间差异无显著性意义（P〉0.05）。治疗12周后相比,两组患者糖化血红蛋白（HbA1c）均显著低于治疗前（P=0.00）,两组间差异有显著性意义（P〈0.05）。那格列奈组患者未出现明显不良反应;阿卡波糖组胃肠道不良反应发生率较高。结论那格列奈（120mg,3次/日）治疗老年2型糖尿病患者疗效肯定,与阿卡波糖（50mg,3～4次/日）降低餐后2小时血糖（2hFPG）、HbA1c疗效相当,不良反应少,依存性良好。

Clinical application of nateglinide and acarbosemonotherapy in newly diagnosed type 2 diabetes mellitus in the elderly

Objective To compare the effective and dependence with nateglinide and acarbosemonotherapy in type 2 diabetes mellitus（T2DM） in the elderly patients.Methods 50 cases were randomized and double-blind divided into two groups,had been measured the fasting blood glucose（FPG）,2-hour postprandial blood glucose（2hFPG）,Glycohemoglobin（HbA1c）and adverse during a 12-week clinical observation.Results FPG were no significant differences between 4 and 12 weeks after treatment（P0.05）.2hFPG were significantly lower than before treatment between 4 and 12 weeks after treatment（P=0.00）,comparison between the two groups was no significant difference.HbA1c were significantly lower than before treatment between 4 and 12 weeks after treatment（P=0.00）,there were no significant difference between the two groups（P0.05）.In the group of nateglinide did not appear adverse reactions,but the incidence of gastrointestinal adverse reactions were higher in the group of acarbosemonotherapy.Conclusion Compared with acarbosemonotherapy（50mg,3 times/day）,the treatment effect of nateglinide（120mg,3 times/day） was positive on type 2 diabetes Mellitus（T2DM） in the elderly patients,had considerable effect on decreasing 2hFPG and HbA1c,was less in adverse reactions and was good at dependence.
