摘    要:目的建立肺炎克雷伯菌随机扩增DNA多态性(RAPD)基因分型的方法,并对新生儿病房呼吸道感染肺炎克雷伯菌进行分子流行病学分析。方法在对肺炎克雷伯菌进行RAPD反应体系优化的基础上,对我院新生儿病房下呼吸道分泌物分离获得的30株肺炎克雷伯菌,提取基因组DNA后进行RAPD分型,同时进行抗生素敏感性试验。结果30株肺炎克雷伯菌RAPD分型可分为14型,分型率100.0%;其中A型6株、B型4株、C型3株、D型3株、E型-H型各2株、I型-N型各1株。药敏结果显示分离菌株多重耐药情况严重,对第三代头孢菌素类抗生素耐药率高达50.0%-73.3%,但对亚胺培南仍高度敏感。结论RAPD可对肺炎克雷伯菌进行分子流行病学研究。新生儿病房内存在肺炎克雷伯菌流行株的交叉感染。

Genotyping of Klebsiella pneumoniae in neonate respiratory infection by random amplified polymorphic DNA technique
Abstract:Objective To establish the random amplified polymorphic DNA(RAPD)genotyping technique for Klebsiella pneurnoniae and to apply it in the molecular epidemiology study.Methods Thirty strains of Klebsiella pneumoniae isolated from neonate sputum samples were collected.DNA extracted from all isolates was amplified by RAPD genotyping technique.Meanwhile.antibiotic susceptibility test was performed.Results Thirty strains of Klebsiella pneumoniae were distinguished into 14 genotypes with a grouping rate of 100.0%.Six.strains had the same genotype A,four strains had the same genotype B,and genotype C and D had three strains respectively.Genotype E,F,G and H had two strains respectively.Antibiotic susceptibility test indicated that the clinical strains of Klebsiella pneumoniae were multi-drug resistant which antibiotic resistance was higher to ampicillin and the third generation cephalosporin(resistance rate accounting for 50.0%-73.3%)but lower to imipenem and amikacin.Conclusion RAPD was a valuable technique for Klebsiella pneumoniae molecular epidemiology study.It can be confirmed there exists several prevalent strains of Klebsiella pneumoniae within newborn ward.
