感染性心内膜炎的流行病学和病原学研究——附118例病例分析

摘    要:目的:为了探讨感染性心内膜炎的流行病学及病原学特点。方法:本文对1978年4月至1997年1月在我院心外科行瓣膜替换术的118例感染性心内膜炎患者的流行病学及病原学特点进行分析。结果:在118例患者中,男性90例,女性28例,平均年龄为34.4a。血培养阳性率为35.6%,瓣膜培养阳性率为41.6%。同时,检测结果显示,瓣膜肠道病毒阳性检出率为41.2%,在血培养阴性者中更高达70.6%。结论:①感染性心内膜炎发病人数有逐年递增趋势,男性患者明显多于女性;②致病微生物以革兰氏阳性菌为主;③肠道病毒亦可能是血培养阴性的感染性心内膜炎的致病因素之一。

Abstract:Object To study the epidemiological and aetiological charactristic of infective endocarditis. Methods One hundred and eighteen patients with infective endocarditis (IE) underwent cardiac operation during April 1978 to January 1997 in our hospital. Results Results showed that the number of the patients with IE increased gradually in the past 20 years and most of them were male; the average age of the patients was 34.4 years old and the most common infective organisms were G~ bacteria. The positive ratio in blood culture was 35.6 % , while was 41.6% in valve culture. The positive ratio of enteriovirus was 41.2% in valver with IE and was 70.6 % in the cultrue-negative valves. Conclusion (l)The number of the patients suffering from infective endocarditis increased gradually in the past twenty years. (2)G~ bacteria were the most common infective organisms. (3)Enteriovirus might also be one of the infective organisms in Culture-negative Endocarditis (CNE) patients.
