缺血性中风患者血糖血脂及内皮素变化与证型的相关性研究

目的:探讨缺血性中风患者(ACI)血管内皮素(endothelin,ET)、血糖、血脂与中医证型的关系。方法:90例ACI患者分为痰热腑实、风火上扰、肝肾阴虚等3个证型组(各30例),并与30名健康体检者作对照,测定ET、空腹血糖(FBG)、血脂含量。结果:痰热腑实、风火上扰、肝肾阴虚证组ET、FBG、低密度脂蛋白胆固醇(LDL-C)、均显著高于健康对照组(P<0.01);肝肾阴虚、风火上扰、痰热腑实组ET、FBG水平依次升高,且与神经功能缺损呈正相关,而LDL-C水平3组比较无显著性差异(P>0.05)。ACI患者的总胆固醇(TC)及总甘油三酯(TG)与对照组比较无显著性差异(P>0.05)。结论:ET和FBG参与了ACI的病理演变过程,是ACI患者机体邪正斗争的病理产物,可作为病情轻重、预后判断及辫证分型的指标。LDL-C虽与ACI的发生有一定的关系,可能是导致其发生的病理基础之一,但TC、TG与ACI及其证型之间的关系仍不明确。

Research on the Relationship Between the TCM Typing of Acute Cerebral Infarction and the Concentration of Endothelin Blood Glucose Serum Lipid

Objective:To explore he relationship between the TCM typing of Acute cerebral infarction and the concentration of endothelin,blood glucose and serum lipid.Methods:90 cases of acute cerebral infarction were divided into three groups,include Type of phlegm-heat viscus-sthenic,Up-stirring of Wind-yang,deficiency of liver-yin and kid-yin(each n=30);While 30 healthy subjects were taken as the control group.Their endothelin,fasting blood glucose and serum lipid were determined.Results:Compared with the control group,the level of ET,FBG,LDL-C raised significantly in the ACI group(P<0.01);the level of ET,FBG in the type of deficiency of liver-yin and kid-yin,Up-stirring of Wind-yang and phlegm-heat viscus-sthenic increased in order;Then the LDL-C concentration among the three groups had insignificant difference(P>0.05).Conclusion:ET and FBG participate the formation and development of ACI,they might infer the type of TCM,outcome and serious or not of ACI;but the relationship between Total-cholesterol,Triglyceride with ACI is unclear.
